한번 찬 기저귀에 소변을 여러 번 싸도 척척하지도 않고 마른 기저귀를 차고 있는 감촉을 주는 수퍼 흡수성 기저귀도 있다. 이런 기저귀를 사서 쓰는 비용과 쓴 후 버릴 때 환경문제가 생기는 것이 단점이다. 그러나 부모들이 아기를 기르는데 훨씬 편리한 것은 큰 장점이다.

기저귀 피부염의 원인
  • 기저귀 피부염의 원인은 아주 많고 기저귀 피부염이 생기는 기전도 복잡하다.
  • 기저귀를 채우지 않으면 기저귀 피부염은 생길 수 없다. 다시 설명하면 기저귀 피부염은 기저귀를 차고 있었기 때문에 생기는 피부병이다. 즉 기저귀 관련성 피부염이다.
  • 기저귀 자체로 생긴 물리적 자극, 기저귀 자체 성분으로 인한 자극, 대소변, 세균 감염, 곰팡이균(진균) 감염, 약물이나 화장품 부작용, 아토피성 피부염, 또는 지루성 피부염 등으로 기저귀 피부염이 생길 수 있다.
  • 이상 여러 가지 원인들 중 한 가지, 두 가지, 또는 그 이상 여러 가지 원인들이 복합적으로 작용해서 기저귀 피부염이 생길 수 있다.
  • 드물게는 매독이나 렛테러-시웨병(Letterer-Siwe disease), 또는 옴 등으로 기저귀 피부염이 생길 수 있다.
  • 기저귀 피부염의 원인에 대해 구체적으로 더 알아보면.
    • 기저귀에 대소변을 쌀 때 그 싼 대소변에서 나온 암모니아 가스, 세균, 산 등으로 기저귀 찬 피부가 자극되면 기저귀 피부염이 생길 수 있다.
    • 대소변을 싼 불결한 기저귀를 오랫동안 계속 차고 있으면 기저귀 피부염이 더 쉽게 생길 수 있다.
    • 아기의 연약한 피부가 찬 기저귀 자체의 물리적 자극을 받을 때 기저귀 피부염이 생길 수 있다(기저귀 참조).
    • 기저귀를 만든 그 성분 자체에서 나온 화학물질 성분으로 피부가 자극을 받을 때도 기저귀 피부염이 생긴다.
    • 부모 형제자매들 중 누구에게 아토피성 피부염이 있거나, 지루성 피부염, 기관지 천식, 또는 알레르기성 비염 등 알레르기성 질환을 앓았던 과거 병력이 있던지, 또는 현재 그런 병을 앓고 있는 병력이 있으면 그 집안의 아기들은 일반적으로 과민한 피부를 갖거나 아토피 체질을 가지고 있을 수 있다. 기저귀 찬 피부가 사소한 자극을 받아도 기저귀 피부염이 더 쉽게 생길 수 있다. 또 그 집안 아기들에게 생긴 기저귀 피부염의 증상 징후는 그렇지 않은 다른 아이들에게 생긴 기저귀 피부염의 증상 징후보다 더 심할 수 있다.
    • 감기 등의 바이러스 상기도 감염을 앓는 아기들이나, 바이러스 위장염 등으로 설사를 하는 아기들에게 생긴 기저귀 피부염은 더 쉽게 생길 수 있고 증상 징후도 더 심할 수 있다. 또 기저귀 피부염이 더 오래 계속 될 수 있다. 아토피성 체질이 있거나, 기관지 천식이나 알레르기성 비염이나 다른 여러 종류의 알레르기성 질환을 앓았던 과거 병력이 있는 아기들의 피부는 일반적으로 더 과민하다.
    • 지루성 피부염이나 아토피성 피부염이나 또는 다른 종류의 알레르기성 피부염이 기저귀 찬 부위에 이미 있는 아기들에게는 기저귀 피부염이 더 쉽게 생길 수 있고 증상 징후도 더 심해지고, 또 자주 생길 수 있다.
    • 지루성 피부염, 아토피성 피부염이나 그 밖의 다른 종류의 알레르기성 피부염이 있을 때 생긴 기저귀 피부염에 세균(박테리아) 감염 또는 곰팡이 감염이 생기면 기저귀 피부염이 훨씬 더 심해진다. 참고로 지루성 피부염을 태열이라고 하며 알레르기성 피부염은 아니다.
    • 위에 열거한 원인 이외, 건선, 포도상 구균 감염, A군 연구균 감염, 옴, 단순포진, 영양부족, 소아학대, 또는 면역 결핍증 등 여러 종류의 원인들에 의해 기저귀 피부염이 생길 수 있다.

기저귀 피부염의 증상 징후
  • 주증도, 원인에 따라 증상 징후가 많이 다르다.
  • 기저귀 피부염은 기저귀를 찬 엉덩이, 하복부, 가랑이, 외음부, 사타구니(샅) 등의 피부의 일부 또는 전체에 생길 수 있다.
  • 경증 기저귀 피부염이 생겼을 때 기저귀 찼던 피부가 햇볕에 덴 1도 화상처럼 빨개진다.
  • 중증 기저귀 피부염이 생기면 기저귀 찬 피부 전체가 붓고, 맑은 진물이 조금 나고, 피가 맺히고, 심하게 가려워서 긁는다.
  • 기저귀 찬 피부의 일부나 전체의 표피가 군데군데 얇게 벗겨질 수도 있다.
  • 기저귀 피부염이 있을 때 신체 어떤 부위에 박테리아 감염병이나 바이러스의 감염병이 있지 않는 한 기저귀 피부염만으로 열이 나지 않는다.
  • 앞서 설명한 것 같이 아토피성 피부염, 기관지 천식 등의 알레르기 질환의 병력이 있는 어린 아기가 본 대소변이 묻은 기저귀를 바로 갈아주지 않으면 기저귀 피부염이 더 쉽게 더 많이 더 심하게 생길 수 있다.
  • 감기나 다른 종류의 바이러스 상·하 기도 감염병, 또는 위장염을 앓는 아기들의 피부는 평소보다 훨씬 더 과민해진다. 이 때 기저귀 피부염이 더 쉽게 생기고, 또 그 증상 징후도 더 심하다.
  • 잠자기 바로 전에 기저귀를 갈아 채울 때는 기저귀 찬 피부가 정상이었지만, 그 다음날 아침 아기에게 기저귀 피부염이 많이 생긴 것을 발견할 때도 있다.
  • 기저귀 피부염에 박테리아 감염이나 진균 감염이 생기면 그 정도가 훨씬 더 심해진다.
  • 일반적으로 과거에 기저귀 피부염을 적어도 한번 이상 앓았던 어린이들은 또다시 기저귀 피부염에 걸릴 가능성이 더 많다.
기저귀 피부염의 진단 
  • 병력, 증상 징후와 진찰소견 등을 종합해 진단할 수 있다.
  • 바이러스 위장염이나 호흡기 감염병이 있을 때, 아기 자신이나 부모 형제자매들에게 알레르기성 질환의 병력이 있든지 아토피성 체질이 있으면 그 집안의 어린이는 기저귀 피부염에 더 잘 걸린다.
  • 따라서 가족 병력이 기저귀 피부염을 진단하는데 큰 도움이 된다.
  • 캔디다 진균 간찰진(Candidal intertrigo), A군 베타 용혈성 연쇄상구균성 간찰진, 농가진, 포도상구균성 간찰진, 헤르페스 바이러스 감염병(단순포진 바이러스 감염병), 영양부족, 소아학대, 면역 결핍증, 옴 등과 감별진단해야 한다.
기저귀 피부염의 치료
  • 기저귀 피부염의 정도와 원인에 따라 치료한다.
  • 기저귀를 채워 주지 않으면 기저귀 피부염이 생기지 않지만 기저귀를 채우지 않고 아기들을 키운다는것이 쉬운 일이 아니다.
  • 기저귀는 일회용 기저귀보다 공기가 더 통하는 천으로 만든 기저귀가 더 좋다.
  • 플라스틱 기저귀 커버는 될 수 있는 한 쓰지 말아야 한다.
  • 어떤 종류의 기저귀를 써도 대소변을 보았을 때는 기저귀를 곧 갈아주는 것이 기저귀 피부염을 예방, 치료하는 비결이다(p.00 기저귀 참조).
  • 일단 기저귀 피부염이 생기면 다 나을 때까지, 또는 피부염의 발병된 후 적어도 하루 이틀 동안은 기저귀를 채우지 않으면 더 잘 치료될 수 있다.
  • 가능한 한 기저귀 피부염이 생긴 부위를 공기에 자주 오랫동안 노출시켜 주어야 한다. 이 때 아기가 춥지 않게 실내 온도를 적절히 조절해야 한다.
  • 다 나을 때까지 필요 이상 비눗물로 씻어주지 말고 필요에 따라 주로 따뜻한 맹물로 씻어주는 것이 좋다.
  • 평소에도 어린이들을 비누로 자주 심하게 씻어주면 피부의 수소지수가 증가되고, 피부 건강에 꼭 필요로 하는 피부지방과 면역체들이 파괴되고 기저귀 피부염이 더 심해지고 악화될 수 있다.
  • 필요에 따라 도브 등 순한 비누를 사용한다. 심한 기저귀 피부염은 의사의 처방에 따라 베타메타손, 엘로콘, 또는 아르스토코트 등의 스테로이드제 크림이나 연고 중 한 가지로 치료한다.
  • 곰팡이균 감염으로 생긴 기저귀 피부염은 마이코스타틴, 로트리민, Vusion(주 성분; 0.25% miconazole, 15% zinc oxide), 또는 마이세렉스 등의 항곰팡이균제 연고로 치료한다.
  • 기저귀 찬 피부에 박테리아 감염으로 생긴 기저귀 피부염은 박트로반 연고와 스테로이드제 연고나 적절한 경구용 항생제 등으로 치료할 수 있다.
  • 기저귀를 채울 때마다 기저귀를 찰 피부에 가루약이나 로션 등을 발라 기저귀 피부염을 예방 치료하기도 한다.
  • 약물성분이 들은 휴지로 기저귀 찬 피부를 씻어 주기도 한다. 이런 것들로 기저귀 피부염을 예방해 주기보다 오히려 기저귀 찬 피부가 자극 받아 때로는 기저귀 피부염이 더 심해질 수 있다.
  • 기저귀 찬 부위가 불결하면 깨끗하고 따뜻한 맹물로 씻어주는 것이 이상적이다. 너무 자주 씻어 주어도 좋지 않다.
  • 기저귀 피부염을 A&D 연고나 데시틴(Desitin) 연고나 크림으로 예방하고 치료를 할 수 있다.
  • 특히 설사를 할 때 항문 주위 피부와 기저귀 차는 피부에 A&D 연고 등을 발라주면 대소변이 그 피부에 직접 묻지 않아 기저귀 피부염이 덜 생길 수 있다.

Causes of diaper dermatitis

Symptoms signs of diaper dermatitis

  • Symptoms and signs vary greatly depending on the main symptom and cause. Diaper dermatitis can occur on some or all of the skin, such as the diapered buttocks, lower abdomen, crotch, vulva, and groin.
  • When your baby has mild diaper dermatitis, his diaper skin turns red like a sunburn. If your baby has severe diaper dermatitis, the entire skin of the diaper is swollen, a little clear swelling, blood formation, and severe itchiness are scratched.
  • Part of or the entire epidermis of the diaper-wear skin may be thinly peeled in several places. When you have diaper dermatitis, you won’t get fever with diaper dermatitis alone unless your baby has a bacterial or viral infection in any part of your body.

Diagnosis of diaper dermatitis

  • It can be diagnosed by synthesizing medical history, symptom signs and examination findings.
  • Children in the household are more susceptible to diaper dermatitis if they have viral gastroenteritis or respiratory infections, have a history of allergic diseases in their babies or their parents, siblings, or have an atopic constitution.
  • Therefore, family history is very helpful in diagnosing diaper dermatitis. Candidal intertrigo, group A beta-hemolytic streptococcal intertrigo, impetigo, staphylococcal intertrigo, herpes virus infectious disease (herpes simplex virus infectious disease), malnutrition, pediatric abuse, immunodeficiency, scabies, etc.

Diaper Dermatitis Treatment

  • Treatment depends on the severity and cause of diaper dermatitis.
  • If your baby doesn’t wear diapers, your baby won’t get diaper rash, but it’s not easy to raise babies without wearing them.
  • Diapers are better made of fabric that breathes more air than disposable diapers. Plastic diaper covers should be avoided as much as possible.
  • Changing diapers soon when your baby has urine, no matter what type of diaper your baby wear, is the secret to preventing and treating diaper dermatitis (see  Diapers).
  • Once diaper dermatitis develops, it can be healed better by not wearing the diaper until it is healed, or for at least a day or two after the onset of dermatitis. As much as possible, the affected area should be exposed to the air as often and for a long time.
  • At this time, the room temperature should be properly adjusted so that the baby does not get cold.
  • It is better not to wash with soapy water more than necessary until it is healed, but mainly with warm plain water as needed.
  • Usually, if children are washed with soap frequently and severely, the hydrogen index of the skin increases, skin fats and immune bodies, which are essential for skin health, are destroyed, and diaper dermatitis may become worse and worse.
  • If necessary, use a mild soap such as dove. Severe diaper dermatitis is treated with one of the steroid creams or ointments, such as betamethasone, Elocon, or Aristocrat, according to the doctor’s prescription.
  • Diaper dermatitis caused by a fungal infection is treated with an antifungal ointment such as Mycostatin, Rotrimine, Vusion (main ingredients: 0.25% miconazole, 15% zinc oxide), or Myserex. Diaper dermatitis caused by a bacterial infection on diaper-filled skin can be treated with Bactroban ointment, steroid ointment, or appropriate oral antibiotics.
  • Whenever the diaper is weared, a powder or lotion is applied to the skin to prevent diaper dermatitis.
  • You can also wash your diaper-wear skin with tissue-containing drug ingredients.

  • Yeonhee It is difficult to give a definite answer without a medical examination. However, when you have any of the following diseases, your buttocks may be crushed. And treatment can be done as follows: “Butt squash” is a type of diaper dermatitis.
  • 1. Atopic dermatitis may cause diaper dermatitis, which may cause the hips to become crushed. In most cases, it is treated with steroid ointments or creams. Then, the cause is removed and the damage is treated.
  • 2.  Diaper dermatitis caused by bacteria may cause “buttocks”. Diaper dermatitis caused by staphylococcal impetigo may result in blisters or pustules with yellow pus. You should see your doctor’s diagnostic treatment soon.
  • 3. A “butt crush” may occur due to diaper dermatitis caused by a fungal infection. It is treated with Mycostatin ointment, etc.
  • 4. A “buttock” may be crushed by diaper dermatitis caused by drugs applied to cold skin. It is not recommended to apply lotion or powder on the diaper area of ​​healthy babies. The skin of healthy babies is beautiful, clean and soft as it is. Applying lotion or powder can damage the skin and cause drug-contact dermatitis.
  • 5. Diaper dermatitis may occur for two or three reasons among 1 to 4 and may cause a “buttocks”.
  • 6 If you don’t change your ears properly and in a timely manner, ammonia from your stool may cause your buttocks to “crush”.
