  • 만삭에 태어난 건강한 신생아들의 대부분에게 생리적 황달이 생후 1~2일부터 정상적으로 나타난다.
  • 건강하게 태어난 신생아가 모유를 먹을 때 모유로 인해 생기는 황달을 ‘모유 황달’이라고 한다.
  • 모유 황달은 아니지만 모유 수유로 생기는 황달을 ‘모유 수유 황달’이라고 한다.
  • 모유 황달이나 모유 수유 황달은 생리적 황달과 조금 다른 점이 있지만 가끔 생리적 황달과 감별 진단하기가 어렵다.
  • 때때로 모유를 먹는 아기에게는 모유 황달, 모유 수유 황달, 생리적 황달이 복합적으로 경미하게 동시 생길 수 있다.
  • 모유 황달은 생후 3~4일부터 나타나기 시작해서 생후 3주내지 생후 2개월까지 지속될 수 있다. 그 원인에 대해서는 여러 가지 학설이 있으나 확실한 원인은 아직 밝혀지지 않았다.
  • 모유를 먹는 신생아들의 대부분은 출생 후 처음 며칠 동안 모유를 충분히 섭취할 수 없어 신생아 생리적 탈수가 생기기 쉽고, 간의 기능이 미숙하고 태아 적혈구 파괴가 가속도로 생기는 등 여러 가지 원인으로 그들에게 생리적 황달이 더 쉽게, 더 심하게 생길 수 있다.
  • 모유 수유 황달이 때로는 더 심해 질 수 있다. 모유 수유 황달은 신생아 생리적 황달과 그 정도가 다른 것이 보통이다. 그 정도에 따라 치료를 달리 한다.
  • 모유 황달에 걸리면 아기의 눈의 흰자위와 피부색이 노랗게 변한다. 좀 더 심할 때는 눈 흰자위와 피부가 더 노래진다. 이 때 신생아의 피부와 눈을 환한 햇빛아래서 잘 관찰하면 노란 피부와 노란 눈 흰자위를 볼 수 있다. 그 밖에 별다른 증상 징후는 없다. 아주 드물게 모유 황달로 핵황달이 생길 수 있다는 문헌상 보고는 있지만 실례를 본적 없다.
  • 증상 징후와 진찰소견을 종합하고, 혈중 빌리루빈 농도를 측정해서 진단할 수 있다.
  • 혈중 빌리루빈 농도가 비정상적으로 더 높고, ABO혈액형 부적합으로 인한 황달이나 다른 원인으로 인한 황달이 없을 때는 모유 황달이나 모유 수유 황달이라고 진단할 수 있다.
  • 모유를 먹을 때 생기는 경미한 모유 수유 황달은 치료하지 않아도 대개 자연히 낫는다.
  • 수유는 평소보다 휴식을 더 취하고 음식물을 더 많이 섭취해서 아기에게 젖을 더 충분히 먹이면 모유 수유 황달이 쉽게 치료된다.
  • 모유 황달이 있는 신생아가 탈수가 되고 혈중 빌리루빈 농도가 비정상적으로 상당히 더 높을 때는 아기를 병원에 입원시키고, 모유 수유를 1~2일 동안 중지하면서 엄마의 젖을 먹이는 대신 보충용 인공영양이나 경구용 5% 포도당 용액 등을 보충용으로 먹이거나 포도당 전해질용액을 정맥주사로 주고 빌리루빈 광선요법 치료를 하면 쉽게 낫는다.
  • 이렇게 치료해서 혈중 빌리루빈 농도가 점점 낮아지기 시작해서 혈중 간접 빌리루빈 농도가 안전수치(보통 10mg)에 근접하거나 그 이하일 때 모유를 다시 먹이기 시작하고 빌리루빈 광선요법을 중지하는 식으로 치료한다.

1월 9일에 자연분만으로 아기 낳았는데요요즘 먹이는 것 땜에 고민이 넘 많아서 이곳에 도움을 청합니다지난주까지 계속 모유수유했었는데아가가 아직까지도 황달이 심하네요수치 15까지 올라가서 병원에서 모유를 중단하고 분유를 먹이라는 말에 이번 주 3일 동안 분유만 먹이고 있습니다담주 토욜까지 약 먹이고 분유먹이고 상태 보자고 하시는데 전 지금 분유 먹이는게 넘 힘들어요아가가 먹긴 잘 먹는데 자꾸만 젖을 찾는 것 같아서 가슴이 아프네요.

① 분유는 시간 간격으로 양만큼만 줘야 하죠?

② 그리고 먹은 지 얼마 안돼서 자꾸 먹으려고 보채면 어떻게 해야하죠?

좋은 질문을 해주셔서 감사합니다자녀의 나이성별과거 병력가족 병력진찰소견임상검사 등 자세한 정보가 더 많이 있을수록 더 좋은 답변을 드릴 수 있지만 주신 정보를 토대로 답변을 드리겠습니다.

신생아에게 생기는 황달을 신생아 황달 또는 신생아 과빌리루빈증이라고도 합니다.

그 원인은 여러 가지입니다.

아무 이상이 없이 생리적으로 생기는 신생아 황달을 신생아 생리적 황달이라고 하고

아무 이상이 없이 모유를 먹는 신생아에게 모유로 생기는 황달을 모유 황달이라고 할 수 있습니다.

모유 황달은 아니지만 모유수유에 관련되어 생긴 황달을 모유수유 황달이라고 합니다.

어떤 이상이 있어 생긴 신생아 황달을 신생아 병적 황달이라고 합니다.

생리적으로 생기는 신생아 황달인지 또는 모유를 먹음으로서 생기는 모유 황달인지 또는 모유수유로 생기는 모유 수유 황달인지 감별하는 것이 중요합니다.

아기의 경우는 모유수유에 관련되어 생기는 모유수유 황달로 진단이 난 것 같습니다.

모유 수유 황달의 원인과 치료에 관해서 말씀드리겠습니다.

모유수유로 인해서 생기는 모유수유 황달은 출생 후 4~7일에 시작해서 길게는 생후 10주 정도까지 계속 될 수 있다.

평균 3주 내에 황달기가 없어지는 것이 보통입니다.

그 원인은 아직도 확실히 모르고 있지만 출생 후 모유수유를 늦게 시작하거나 모유수유 양이 불충분하거나

모유수유의 횟수가 매번 모유수유를 할 때 충분한 시간을 소요하지 않거나

모유수유 하는 방법이 옳지 않거나

아기가 모유를 제대로 수유하지 않거나

미숙 신생아로 태어나거나 아기가 어디가 아플 때나 수유모가 산후 블루나 산후 우울증에 걸려 우울하거나

또는 충분히 휴식을 취하지 못하거나

영양섭취를 충분히 취하지 못하면 신생아가 탈수될 수 있고 영양부족이 생겨 모유수유에 관련된 모유 황달이 생기기 쉽습니다.

다행히도 모유수유로 인한 모유수유 황달로 핵황달이 생긴 예도 없고 저 자신도 그런 환자를 본적이 없습니다.

그러나 간접형 빌리루빈의 농도가 혈중 15~20mg 이상 증가되지 않게 치료하는 것이 보통입니다.

모유수유로 인해 황달이 생겼을 때 일반적으로 간접형 빌리루빈의 농도가 혈중 15~20mg 이상 더 올라가지 않도록 여러 가지 방법으로 치료합니다.

2~3시간 간격으로한 번에 적어도 15~30분 간 모유수유하고,

수유모는 충분히 수면을 취하고,

충분히 먹고 정신적 육체적으로 충분히 휴식을 취하면

모유의 분비가 잘 되고 아기가 젖을 많이 먹으면 빌리루빈이 더 이상 증가되지 않든지 아주 조금 증가됨으로 모유수유를 일시적으로 중단할 필요가 없이 계속 모유수유를 할 수 있습니다.

그러나 혈 중 빌리루빈의 농도가 계속 증가되거나 짧은 시간 내에 비정상적으로 더 속히 증가될 때는 1~3일 동안 모유수유를 중단하고 때로는 빌리루빈 광선요법으로 치료도 하고 인공영양(20칼로리/30cc) 90~150cc를 매 2~3시간 간격으로 먹이면 모유수유로 인한 모유수유 황달이 좋아지고 혈 중 간접형 빌리루빈의 혈중농도 수치가 10~15mg 이하로 떨어지게 됩니다.

이때부터 모유수유를 다시 곧 시작하면 빌리루빈의 수치가 다시 조금 반등되어서 증가 될 수는 있으나 건강 상 유해할 정도로 더 많이 증가되지 않는 것이 보통입니다또는 빌리루빈이 한번 안전치 이하로 떨어진 후에는 계속 점점 떨어져서 간접형 빌리루빈의 혈중 농도가 정상치로 돌아가는 것이 보통입니다.

5% 경구용 포도당액이나 신생아용 수액은 의사의 지시가 없는 한 먹이지 않는 것이 좋습니다.

아기의 경우모유수유를 다시 시작하고 인공영양을 더 이상 먹이지 말고 하루 정도 기다렸다가 빌리루빈 혈 중 농도를 다시 검사해보면 하루 동안에 빌리루빈이 어느 정도로 증가되었는지 알 수 있고 또 앞으로 매일 어느 정도 더 증가될 수 있는지 예측할 수 있습니다.

아기는 생후 점점 더 성장하고 나이(일수)를 더 먹음에 따라 간장의 기능도 더 성숙해지고 그에 따라 빌리루빈 생성 속도가 점점 더 감소되고 황달이 정상적으로 점점 적게 생기는 것이 보통입니다.

그럼으로 앞으로 빌리루빈 농도가 이전보다 점점 천천히 증가되든지 감소될 것이라고 예측할 수 있습니다.

이런 저런 이유로 모유수유를 다시 시작해도 좋을 것 같습니다.

그리고 아기에게 영아 산통이 있는지 인공영양이 체질에 잘 맞지 않는지도 의심할 수 있습니다.

아기를 보지 않고 진찰하지 않고 이렇게 인터넷으로 상담을 드리지만 모든 치료는 단골 청소년과 의사의 지시에 따라 이루어져야 합니다.

먼저도 말씀드린바와 같이 천 페이지를 가지고 모유수유에 관한 정보를 한 책에 다 쓰더라도 아마도 부족할 정도입니다.

그 이유는 그때그때 따라 모유를 수유하는 방법이 다를 수 있기 때문입니다.

그리고 모유수유 기술이란 말도 있습니다.

모유수유 하는 데도 상식이 통해야 하는 것입니다.

다시 말씀드리면 각 아기에 따라 또 각 수유모에 따라 그때그때 상황에 따라 모유수유 방법이 조금씩 달리할 수 있는 것입니다.

누워서 수유 할 수 있습니다.

그러나 누워서 모유수유를 할 때 수유 중 엄마가 잠들면 아기를 누를 수 있기 때문에 문제가 생길 수 있습니다.

누워서 모유수유 하는 것은 일반적으로 많이 권장하지 않습니다.

모유수유를 하다가 수유모가 잠들 때 아기를 한쪽으로 옮겨 뉠 수 있는 사람이 옆에 지키고 있을 때는 편히 누운 자세로 모유수유를 할 수 있습니다.

트림을 시켜도 젖을 넘기는 아기도 있고

가스가 조금도 나오지 않는 아기도 있습니다.

트림을 한 번 안 시켜도 젖을 한 번도 넘기지도 않고 게트림도 조금도 하지 않고 구토도 한 번도 하지 않고 잘 자라는 아기도 있습니다.

그럼으로 트림을 꼭 시킬 필요는 없습니다.

모유수유로 인한 모유수유 황달은 사실상 치료를 하지 않아도 문제가 생기지 않을 수 있지만 혹시라도 치료를 하지 않으면 핵황달 등이 생길 수 있을까 염려되기 때문에 치료합니다.

다른 원인으로 생긴 황달과 감별해야 합니다.

모유로 인한 경한 모유황달은 두 달 정도 계속 될 수 있습니다.

그 진단이 맞았으면 황달기가 다소 있더라도 걱정하실 필요가 없습니다.

보통 영유아들의 헤모글로빈 농도수치가 12~15정도 될 수 있지만 신생아의 헤모글로빈의 농도수치는 18 정도로 높은 것이 정상입니다.

그런 이유로 초기의 신생아의 피부는 정상적으로 더 붉거나 빨갈 수 있습니다.

거기다가 아기가 울면 얼굴 피부에 있는 모세혈관 내 피가 더 많이 차서 더 붉은 피부색이 되는 것입니다.

아기의 경우는 정상적인 반응으로 그렇게 붉은 것 같습니다.

모유를 먹는 어떤 아기들은 밤에 5~6시간 정도 깨지 않고 정상적으로 잠을 자고 낮에는 자주 모유수유를 하는 경우도 있습니다.

그렇게 정상적으로 잠자고 체중이 정상적으로 증가되면 아기가 밤에 좀 길게 잠을 많이 잔다고 해서 꼭 깨워 젖을 먹이지 않아도 됩니다.

아시겠지만 진찰 하지 않고 말씀드리기 때문에 제 설명이 꼭 들어맞지 않을 수 있습니다.

There are many kinds of jaundice occurred in newborns as followings

  • Physiological jaundice,
  • Jaundice due to ABO blood type incompatibility,
  • Jaundice due to Rh blood type incompatibility,
  • Jaundice due to various types of infectious hepatitis, such as hepatitis B,
  • Breast milk jaundice,
  • Several types of jaundice can occur,
  • including breastfeeding jaundice.
  • In most of the healthy newborns born at full term, physiological jaundice appears normally from 1 to 2 days after birth.
  • Jaundice caused by breast milk when a healthy newborn baby feeds on breast milk is called ‘breast milk jaundice.’
  • Although not breastfeeding jaundice, jaundice caused by breastfeeding is called “breastfeeding jaundice.”
  • Breast-feeding jaundice or breastfeeding jaundice differs slightly from physiological jaundice, but it is sometimes difficult to differentiate and diagnose physiological jaundice.
  • Occasionally, breastfeeding babies may have a combination of mild breastfeeding jaundice and physiological jaundice.
  • Breast milk jaundice begins to appear from the 3rd to 4th day of life and can last from 3 weeks to 2 months of age.
  • There are various theories about the cause, but the definite cause has not yet been identified.
  • Most of the breastfeeding newborns do not have enough breast milk during the first few days after birth, which is more prone to neonatal physiological dehydration, poor liver function and accelerated fetal red blood cell destruction.
  •  It can be worse. Breastfeeding jaundice can sometimes get worse. Breastfeeding jaundice is usually different from neonatal physiological jaundice.
  • The treatment is different depending on the degree.
  • If you have breast milk jaundice, the whites and skin color of your baby’s eyes turn yellow. In more severe cases, the whites of the eyes and skin become more yellow.
  • At this time, if you observe the newborn’s skin and eyes in bright sunlight, you can see yellow skin and yellow eyes.
  • There are no other signs, symptoms.
  • Rarely, there are reports in the literature that breast milk jaundice can lead to nuclear jaundice, but I have never seen an example.
  • It can be diagnosed by synthesizing symptoms, signs and examination findings and measuring the concentration of bilirubin in the blood.
  • If the blood bilirubin level is abnormally higher and there is no jaundice due to ABO blood type incompatibility or other causes, it can be diagnosed as breast milk jaundice or breastfeeding jaundice.
  • Mild breastfeeding jaundice caused by breastfeeding usually heals naturally without treatment.
  • Breastfeeding jaundice is easily cured by taking more rest than usual and eating more food to give your baby more milk.
  • When a newborn baby with breast milk jaundice is dehydrated and the blood bilirubin level is abnormally significantly higher, the baby is admitted to the hospital, and breastfeeding is stopped for 1 to 2 days and instead of feeding the mother’s milk, supplementary artificial nutrition or oral 5% glucose.
  • It is easily healed by feeding a solution, etc. as a supplement, or by giving a glucose electrolyte solution intravenously and treating with bilirubin phototherapy.
  • With this treatment, the concentration of bilirubin in the blood begins to gradually decrease, and when the indirect bilirubin concentration in the blood is close to or below the safe level (usually 10 mg), breastfeeding begins again and bilirubin phototherapy is stopped.

  • Breastfeeding every 2 to 3 hours, at least 15 to 30 minutes at a time, The nursing mother gets enough sleep, If you eat enough and get enough rest mentally and physically, If breast milk is well secreted and the baby is breastfed a lot, bilirubin is no longer increased or only slightly increased so that breastfeeding can continue without the need to temporarily stop breastfeeding.
  • However, when the concentration of bilirubin in the blood continues to increase or increases abnormally faster within a short period of time, breastfeeding is stopped for 1-3 days, sometimes treated with bilirubin phototherapy and artificial nutrition (20 calories/30cc) 90-150 cc. If fed every 2-3 hours, breastfeeding jaundice due to breastfeeding improves, and the blood level of indirect bilirubin in the blood drops below 10-15mg. If breastfeeding is resumed from this point on, the level of bilirubin may rebound a little again and increase, but it is common that it does not increase as much as it is harmful to health.
  • Or, once bilirubin falls below a safe level, it continues to fall gradually, and blood levels of indirect bilirubin return to normal. It is not recommended to give 5% oral glucose solution or newborn sap unless directed by a doctor. In the case of babies, restart breastfeeding, stop feeding any artificial nutrition, wait a day, and then check the bilirubin blood level again to determine how much bilirubin has increased during the day, and to predict how much more it can increase each day in the future. can.

Breastfeeding is also like the art of breastfeeding, drawing a picture. In addition, breastfeeding is ninjutsu and belongs to medicine.

As mentioned above, even if you have a thousand pages and write all the information about breastfeeding in one book, it is probably not enough. This is because breastfeeding methods may vary from time to time. And there is also a term for breastfeeding techniques.

Common sense should also be used to breastfeed. In other words, the breastfeeding method may be slightly different for each baby and for each nursing mother. You can lie down and feed. However, when lying down and breastfeeding, problems can arise because the mother can press down on her baby if the mother falls asleep during lactation

Lying down breastfeeding is generally not recommended. During breastfeeding, when the nursing mother falls asleep, the baby can be shifted to one side and a person who can lie next to her is kept by her, allowing you to breastfeed in a relaxed position.

Some babies pass their breasts even if they burp Some babies don’t have any gas. Some babies don’t belch once, don’t pass milk, don’t get any gettering, don’t vomit, and grow up well. So there is no need to trim.

Breastfeeding jaundice caused by breastfeeding may not cause problems without treatment, but it is treated because it is concerned that nuclear jaundice may occur if it is not treated.

You have to differentiate it from jaundice caused by other causes. Mild breast milk jaundice can last up to two months. If the diagnosis is correct, you don’t need to worry if you have some jaundice.

Normally, infants’ hemoglobin levels can be 12 to 15, but newborns’ hemoglobin levels are normally as high as 18.

For that reason, the skin of an early newborn may normally be redder or redder. In addition, when the baby cries, the blood in the capillaries on the skin of the face fills up more, resulting in a redder complexion.

In the case of babies, it seems to be that red as a normal reaction. Some breastfed babies sleep normally without waking for 5 to 6 hours at night and breastfeed frequently during the day.

If you sleep normally and gain normal weight, you don’t have to wake up and breastfeed just because your baby sleeps a little longer at night. As you may know, my explanation may not be true because I am speaking without a medical examination.
