신생아 두개골 골절의 원인

특히 난산으로 태어난 신생아들의 두개골이 골절될 수 있다.
드물게 자연 분만으로 순산한 신생아의 머리가 질 산도에 세게 눌려 두개골이 골절되기도 한다.
겸자로 분만할 때도 두개골이 골절될 수 있다.
분만과는 아무 관계없이 안전사고 등으로 신생아의 두개골이 골절되는 경우도 있다.

두개골 골절의 정도, 부위, 원인, 합병증의 유무 등에 따라 증상 징후가 다르다.
두개골이 경미하게 골절된 경우, 증상 징후가 거의 없을 수 있다.
드물게 갓 태어난 아기가 어떤 병을 앓을 때 그 병을 진단하기 위하여 두개골 X선 사진검사 등을 할 때 두개골이 골절된 것을 우연히 발견할 수 있다.
신생아 두혈종과 경미한 두개골 골절이 동시 생길 수 있다.
두개골이 심하게 골절될 때는 두개골 골절과 두개강 내 출혈이 동시에 있을 수 있다.
이 경우, 두개골 골절로 생긴 증상 징후보다도 두개강 내 출혈로 인한 증상 징후가 주로 나타나는 것이 보통이다.

신생아 두개골 골절의 진단 치료

병력, 증상 징후, 진찰소견 등을 종합하여 이 병이 의심되면 두부 CT 스캔 검사, 두부 MRI, 두부 X선 사진, 두부 초음파 사진 검사 등으로 진단할 수 있다.
신생아의 두개골이 아주 심하게 골절되면 수술로 적절히 치료한다.
신생아의 두개골이 아주 경미하게 골절되면 그대로 두고 관찰적 치료를 하면 저절로 낫을 수 있다.

Causes of skull fractures in newborns

In particular, the skull of newborns born with difficult delivery can be fractured. Rarely, the head of a newborn baby who has passed through natural labor is pressed hard against the vaginal birth canal, causing a skull fracture.

The skull can also be fractured when delivered with forceps.

Symptoms and signs are different depending on the severity of the skull fracture, the site, the cause, and the presence or absence of complications.
If the skull is slightly fractured, there may be few signs, symptoms.
Rarely, when a newborn baby suffers from a certain disease, a fracture of the skull can be accidentally discovered when a skull x-ray is performed to diagnose the disease. Neonatal cephalohematomas and minor skull fractures can occur simultaneously. There may be both a skull fracture and intracranial bleeding when the skull is severely fractured.
In this case, symptoms due to intracranial bleeding are more common than those caused by skull fracture.

Diagnosis, treatment of neonatal skull fractures

If the disease is suspected by combining the medical history, symptoms, and examination findings, it can be diagnosed with a head CT scan, a head MRI, a head X-ray, and a head ultrasound.
If the newborn’s skull is severely fractured, it is appropriately treated with surgery. If a newborn’s skull has been fractured very slightly, it can heal on its own if it is left untouched and subjected to observational treatment.
