아프가 점수(아프가 점수 평가 법), Apgar scoring

갓 태어난 신생아의 호흡 상태, 심장 박동 상태, 피부색, 근육의 힘, 신경 반사 5가지의 건강 상태, 를 체크해서 결과를 0, 1, 2 수치로 매겨 갓 태어난 신생아의 총체적인 건강 상태의 정도를 객관적으로 알아보는 방법을 아프가 점수, 아프가 채점법, 아프가 점수 평가법 또는 아프가 스코어(Apgar score)라고 한다.
미국 마취학 전문의 아프가(Apgar)박사가 아프가 스코어를 연구 발표했다.
갓 태어난 신생아의 5가지 총체적 건강 상태의 정도를 객관적으로 체크해서 점수로 채점했다.
그래서 출생 후 1분됐을 때와 5분 됐을 때 신생아의 5가지의 건강 상태의 정도를 객관적으로 알아볼 수 있는 가장 좋은 신생아 임상적 이정표가 바로 아프가 점수이다.
처음 연구 발표한 후 40여년이 지났지만 아직도 임상 신생 소아과에서 널리 쓰이고 있다. 아프가 박사에게 축하한다.
생후 1분과 5분에 갓 태어난 신생아의 심장 박동 상태, 호흡 상태, 근육의 이완과 강직의 정도, 카데터로 비강 점막층이나 인두 점막층을 자극할 때 신경 반응의 정도, 피부의 색이 핑크 색인지 청색인지 체크해서 각 5가지 항에서 얻은 건강 상태의 정도에 따라 0~2점을 먹인다.
그 채점에서 얻은 각각 점수를 총합계해서 나온 총 아프가 점수로 생후 1분과 5분에 갓 태어난 신생아의 총체적 건강상태를 객관적으로 알아볼 수 있다.
위 다섯 종류의 건강상태를 체크해서 얻은 각 점수를 종합해서 생후 1분과 5분의 아프가 점수의 총점을 계산한다.
아프가 점수의 총점이 얼마인가에 따라, 생후 1분, 5분에 갓 태어난 신생아의 건강상태가 얼마나 좋았었는지 나빴었는지를 객관적으로 3자들도 알아볼 수 있다.
그러나 아프가 점수의 총점에 따라, 갓 태어난 아기에게 심폐 소생술을 시작해야 되는지, 시작 하지 않아도 되는지 결단을 내리는 데는 부적절하다.
갓 태어난 신생아 심폐 소생술은 생후 수십 초 이내 시작해야 적절한 때도 있기 때문이다.
갓 태어난 아기의 이상적인 생후 1분 아프가 총점은 8∼10점이다. 생후 5분의 이상적인 아프가 총점은 역시 8∼10점이다.
예를 들면 생후 1분에 갓 태어난 아기의 피부색이 푸르스름하면 0점, 심장 박동이 1분에 120 정도이면 2점, 숨은 쉬지만 잘 쉬지 못하면 1점, 팔 다리 근육에 힘이 많이 있으면 2점, 콧구멍 통한 비강 속 카데터 자극을 받을 때 재채기를 하든지 얼굴을 찡그리면 2점을 줄 수 있다.
이 경우 이상 갓 태어난 아기의 5종류의 건강상태를 체크해서 얻은 각 아프가 점수를 종합한 총점이 7이 된다. 갓 태어난 아기의 1분의 아프가 점수는 7이란 뜻이다.
갓 태어난 아기의 생후 5분에 같은 요령으로 아프가 점수의 총점을 채점한다.
이 때 얻은 총 점수가 생후 5분의 아프가 점수의 총점이 된다.
갓 태어난 신생아들의 1분 아프가 점수의 총점이 5점이거나 그 이하이면, 생후 1분일 때 그 아기의 총체적 건강 상태가 그렇게 좋지 않았다는 것을 의미한다.
건강 상태가 계속 좋지 않으면 생후 10분에 아프가 점수의 총점을 측정할 수 있다.
이와 같이 아프가 점수는 갓 태어난 신생아의 건강 상태가 어느 정도이었는지 객관적으로 알아보는 데 매우 중요한 자료가 된다.

생후 1, 5, 10분 아프가 총점이 각각 8, 10, 10인 예이다.
이 예에서 생후 1분에 갓 태어난 신생아의 피부가 창백했었기 때문에 피부색의 건강 상태에서 1점을, 호흡 곤란이 약간 있었기 때문에 호흡의 건강 상태에서 1점을 주었다
분만한 후 내 아기의 아프가 점수가 얼마였는지 알아보는 것도 좋은 일이다.

Apgar scoring (Apgar scoring method) 아프가 점수(아프가 점수 평가 법)

The health status of the newborn baby’s breathing status, heart rate, skin color, muscle strength, and nerve reflexes is checked and the results are evaluated as 0, 1, and 2 values to objectively determine the degree of the newborn’s overall health. The method of finding out is called the Apgar score, the Apgar scoring method, the Apgar score evaluation method, or the Apgar score.
Dr. Apgar, an American anesthesiologist, published a study on the Apgar Score.
The five levels of the overall health of newborn babies were objectively checked and scored.
So, the best neonatal clinical milestone that can objectively determine the degree of a newborn’s five health conditions at 1 minute and at 5 minutes after birth is the Apgar score.
Although 40 years have passed since the first study was published, it is still widely used in clinical newborn pediatrics.
Congratulations to Dr. Apgar. Newborn babies at 1 and 5 minutes of age, heart rate, breathing, muscle relaxation and stiffness, nerve response when stimulating the nasal mucosa layer or pharyngeal mucosa layer with a catheter, and the color of the skin is pink or blue.
Check for awareness and feed 0 to 2 points depending on the level of health obtained in each of the 5 items. With the total Apgar score obtained by summing each score obtained from the scoring,
it is possible to objectively find out the overall health of newborns born at 1 minute and 5 minutes after birth. The total score of 1 minute and 5 minutes after birth is calculated by synthesizing each score obtained by checking the above five health conditions.
Depending on the total Apgar score, three people can objectively determine how good or bad the health of newborn babies at 1 and 5 minutes of age was good or bad.
However, based on the total Apgar score, it is inappropriate for deciding whether to start CPR on a newborn baby or not.
This is because there are times when it is appropriate to start CPR within a few tens of seconds after birth. The ideal 1 minute after birth for a newborn baby is a total score of 8-10 points.
The ideal Apgar total score of 5 minutes after birth is also 8-10 points.
For example, if the skin color of a newborn baby is bluish at 1 minute of age, 0 points, 2 points if the heart rate is around 120 per minute, 1 point if breathing but unable to rest well, 2 points if there is a lot of strength in the muscles of the arms and legs,
You can give 2 points by sneezing or frowning when you are irritated by the nasal catheter through your nostrils. In this case, the total score of each Apar score obtained by checking the five kinds of the health status of the abnormally newborn baby becomes 7.
One minute for a newborn baby, an Apgar score is 7.
At 5 minutes after the birth of a newborn baby, the total score of the Apgar score is scored using the same technique.
The total score obtained at this time is the total score of the Apgar score of 5 minutes after birth.
If a newborn baby’s 1-minute Apgar score totals 5 or less, it means that the baby’s overall health was not so good at the first minute of life. If your baby still in poor health, he can get a total score on your Apgar score at 10 minutes after birth.
As such, the Apgar score becomes very important data for objectively examining the health status of a newborn baby.

This is an example where the total score of 1, 5, and 10 minutes after birth is 8, 10 and 10, respectively. In this example, 1 point was given for the healthy condition of the skin color because the skin of a newborn baby was pale at 1 minute after birth, and 1 point was given for the health condition of breathing because of a slight difficulty in breathing. It’s also a good idea to find out what my baby’s ill score was after delivery.
